RESUMO

Objective:To investigate the therapeutic effect of skull drilling and/or grinding combined with artificial dermis and vacuum sealing drainage in repairing scalp defects with skull exposure.Methods:From October 2014 to May 2018, 18 patients with scalp defect and skull exposure were treated in the Department of Burn and Plastic Surgery, the Second Clinical College of North Sichuan Medical College, including 10 males and 8 females, with an average age of 64 years (range, 34-86 years). The patients were divided into two groups: group A (by drilling skull or/and grinding combined with artificial dermis cover and vacuum sealing drainage plus two split thickness skin graft repair) and group B (by drilling skull or/andgrinding combined with artificial dermis cover plus two covering leather grinding stage split thickness skin graft repair), 9 cases in each group. The head wound granulation tissue, postoperative complications, skin graft survival rate and wound healing time were compared between the two groups. Vancouver scar assessment scale (VSS) was used to evaluate the wound healing in the two groups.Results:The time of granulation cultivation in group A and group B was (16.44±1.42) days and (29.11±13.32) days, the difference was statistically significant ( P<0.05); The wound healing time of group A and group B was (26.00±3.32) days and (40.67±14.37) days, the difference was statistically significant ( P<0.05); The postoperative complications of group A and group B were 1 case and 5 cases respectively, the difference was statistically significant ( P<0.05). The skin graft survival rates of group A and group B were (97.11±3.44)% and (95.00±4.74)%, the difference was not statistically significant ( P>0.05); The wound scar VSS scores of group A and group B were (7.67±1.32) points and (8.78±1.99) points, the difference was not statistically significant ( P>0.05). Conclusions:By drilling skull and/or grinding combined with artificial dermis cover and vacuum sealing drainage and two stage split thickness skin graft for repairing scalp defect with skull exposure wound can not only better scalp defect with skull exposure wounds, and reduce the postoperative complications, and significantly accelerate wound healing, but also can effectively improve the quality of wound healing, which is worthy of clinical application.

OBJECTIVE: Microvascular endothelial dysfunction, which is at the early stage of atherosclerosis, precedes macrovascular endothelial dysfunction. The study is aimed to investigate the mechanism underlying the improvement of microvascular endothelial dysfunction by exercise and diet in obese adolescents. METHODS: A quasi-randomized study was carried out with 2 cohorts: the experimental group (57 obese male adolescents; age: 15.38 ±â€¯2.82 years, BMI: 33.21 ±â€¯4.23 kg/m2) completed a 6-week exercise program with dietary intervention, and control group (10 normal weight adolescents; age: 15.38 ±â€¯2.82 years, BMI: 23.21 ±â€¯4.23 kg/m2) maintained sedentary. Clinical characteristics, circulating NO, ET-1 and microRNA-126 (miR-126) levels were measured before and after 6 weeks. The Reactive Hyperemia Index (RHI) was measured using EndoPAT-2000 system. RESULTS: After 6-weeks intervention, obese adolescents' body circumferences and glucolipid metabolism are significantly improved. RHI (p < 0.01) and serum levels of NO/ET-1 (p < 0.01) are significantly increased, while microRNA-126 significantly decreased (p < 0.01). ΔMiR-126 were positive correlated with ΔBMI (r = 0.60, p < 0.01), ΔRHI (r = 0.69, p < 0.05), and ΔNO/ET-1 (r = -0.68, p < 0.05). CONCLUSIONS: Combination of exercise and diet control can effectively improve glycolipid metabolism of obese adolescents, and thus their microvascular endothelial function, which might be related to changes in serum miRNA-126.

RESUMO

The widespread application of next generation sequencing (NGS) in clinical settings has enabled testing, diagnosis, treatment and prevention of genetic diseases. However, many issues have arisen in the meanwhile. One of the most pressing issues is the lack of standards for reporting genetic test results across different service providers. The First Forum on Standards and Specifications for Clinical Genetic Testing was held to address the issue in Shenzhen, China, on October 28, 2017. Participants, including geneticists, clinicians, and representatives of genetic testing service providers, discussed problems of clinical genetic testing services across in China and shared opinions on principles, challenges, and standards for reporting clinical genetic test results. Here we summarize expert opinions presented at the seminar and report the consensus, which will serve as a basis for the development of standards and guidelines for reporting of clinical genetic testing results, in order to promote the standardization and regulation of genetic testing services in China.

RESUMO

AIM To develop an HPLC method for the simultaneous content determination of berberine hydrochloride,jatrorrhizine hydrochloride,palmatine hydrochloride,kaempferol,luteolin and apigenin in Qingre Zhili Pills (Berberidis Radix and Portulacae Herba).METHODS The analysis of methanol extract of this drug was performed on a 35 ℃ thermostatic Agilent TC-C18 column (4.6 mm × 250 mm,5 μm),with the mobile phase comprising of acetonitrile-0.02% potassium dihydrogen phosphate flowing at 0.9 mL/min in a gradient elution manner,and the detection wavelengths were set at 265 nm and 360 nm.RESULTS Six constituents showed good linear relationships within their own ranges (r≥0.999 3),whose average recoveries were 97.16%-99.90% with the RSDs of 0.62%-1.48%.CONCLUSION This simple method can be used for the rapid quality control of Qingre Zhili Pills.

RESUMO

Objective To observe the efficacy of percutaneous hepatic cryoablation for the treatment of primary or meta-static hepatic malignancies (<5 cm in diameter).Methods A total of 31 patients (39 tumors <5 cm in diameter) were treated with argon-helium cryoablation system under the guidance of CT or ultrasound.Results Tumor ablation range was 90%-100% in 39 lesions,including 69.23% (27/39) complete ablation.The 1- and 2-year survival rate was 90.32% (28/31) and 61.29% (19/31),respectively.No bleeding and injury of blood vessel or bile duct was noted.Complications of cryoablation included intraoperative shivering in 4 (12.90%) patients,postoperative fever (37.12-38.25℃ in 7 (22.58%) patients and hepatic pain in 6 (19.36%) patients.One patient had severe pain relief until 2 h after cryosurgery with ice-cold skin temperature and stable life index,analgesic had little effect,and no bleeding was found on CT image.Other patients had slight or moderate pain and remained untreated.Conclusion Percutaneous targeted argon-helium cryoablation is a feasible and safe technique in the treatment of small primary or metastatic hepatic malignancies not suitable for resection.

RESUMO

Objective:To study an identifying model of HPLC fingerprint for the overall quality control of traditional Chinese medicine. Methods: A HPLC method is used to establish the standard fingerprint as well as sample fingerprint first. Then through the principles of numerical taxonomy and statistics, the similarity of the sample to the standard is tested by coefficients of cosine and distant, which are calculated with computer. Results: The combination of the two coefficients of cosine and distant can test the similarity of the sample to the standard correctly and accurately. Conclusion: The coefficients along with HPLC fingerprint can be applied to the on-line control in workshop to ensure the products' quality of all batches.

AIM: To establish a fingerprint analytical method for ensuring the quality of Banlangen Granule (Isatis indigotica Fort). METHODS: According to pre column derivatization HPLC method, gradient elution with sodium acetate buffer (pH6.4) acetonitrile (95∶5) in a BDS C 18 column, was applied to detect amino acid at UV 360nm; and the coefficients of cosine of include angle and distance were employed to judge the quality similarity of the samples. RESULTS: The HPLC method is accurate and reproducible and makes almost all the peaks separated; and the two coefficients can show the similarity of each sample. CONCLUSION: The developed method is simple, accurate and operable and can efficiently control the quality of Banlangen Granule.

RESUMO

Objective: To study the changes of andrographolides in the drying process of the extract of Andrographis paniculata Nees. Methods: HPLC method was applied to analyze andrographolide and 14 deoxy 11, 12 didehydroandrographolide in the process. Results: The content of andrographolide descended rapidly in the whole drying process, while the content of 14 deoxy 11, 12 didehydroandrographolide ascended at first 12 hours, declined in content was slowly to follow. Conclusion: Baking temperature is not the only main factor to stimulate the transformation of andrographolide and 14 deoxy 11, 12 didehydroandrographolide.
